is it one use only? or can i use it multiple times

it's infinite use. you can just keep it locked in your inventory and it will continue doing its job as long as you leave it there.

Additional advice: get both the trap and the repellant! The Trap gives you a 50% chance of obtaining the item mentioned in the Lizard Random event. The Repellant gives you a 25% chance of obtaining the item mentioned in the Lizard Random event. But BOTH of them locked in your inventory sets it to 100%!

but if you also have the repellant you can't capture the comatose lizard, so it's advisable to only add the repellant after you've gotten all tiers of that achievement (assuming achievements are something you want).
