Everything is filled out correctly, but nothing happens when I click Submit. I have already cleared both caches, restarted PC, etc. I have tried Chrome Version 91.0.4472.164 and Firefox Version 89.0.2 without success since last week.

Have you tried clearing your cache "for all time"? Some Internet browsers only clear 2 weeks' worth by default, so you may need to clear your cache "for all time" to see if that fixes it!
