EDIT nevermind I'm an idiot, please delete ASAP

I keep "earning" the same quest achievements (eg the Librarian achievement, Split Personality: Mori/Nori) over and over every time I complete six or seven quests in a row. None of them actually show up in my achievements. Is this happening to anyone else?
