Can't Submit Slots for a CW

Was trying to submit a new CW and I can't fill in the slots. I'm getting this on the submission page:

No form appears. Anyone else getting this? -_-

As of this morning, this is still broken. Trying to get a birthday item onsite, would appreciate some answers. Thank you!

Okay so it still works on Chrome and I was able to submit using that but is broken for Palemoon still.
