I'm pretty sure it's a Rocky Horror Picture Show reference, which is why it looks that way.

edit: Upon looking at the description, it's DEFINITELY a Rocky Horror ref. Looking up images all called it a lab coat too, despite it being a surgical gown.

It is not mislabeled because it is used in a mad scientist laboratory while doing a (rather gruesome) operation. Nothing actually scientific or medical is happening because it's horror.

I understand it's a reference to the movie, but if the character is doing an operation while wearing it then wouldn't it be a surgical gown either way? It's styled as one and being used as one.
Yeah, it's still technically a surgical gown in the movie right? Not everything a mad scientist wears is automatically a lab coat. Do they call it a lab coat in the movie? I don't remember.
