I just realized my 3 year Subeta anniversary was on the 21st, but I've not gotten the next achievement tier or Loyalty Box, and the achievement page still says I'm at 2 years. Anything I can do to fix this?

Possibly a dumb question, but did you go to the User Dashboard and refresh?
