The filename is not empty - the CI file is 64x64. The overlay is not empty either - the file is 125x250.
I double-checked both of my files to verify.
I had to reopen the submission page and redo everything, at that point it worked.
Also happened when trying to submit a second item:

Also, just submitted another CW and was directed to an Internal Error 500 page. Hit the back button, clicked it again, and it went through fine the second time around.
