I get frequently false error reports when I do the quests. It appears very often when I finish a quest. This happen by all quest givers.

Oh, No! You aren&;t on a quest

That's not the case. I do the quests accurately.

I've had that happen before, and I feel like it's likely a double-click or something, although for the last few days, I've had issues with 404's eating my items and making me have to get them again for quests.

Those are usually double-clicks, one of the computers i use has a mouse that is notorious for doing that. However, I'm on my normal computer and i just got an "oh no" error from the fragmentizer that i'm concerned about. It said it didn't take my items, but my crystals are gone, and i didnt pay attention to how many i had in there previously so i don't know if it counted or not. :/

If you are worried a UA usually has logs of that and can look into it if you file a ticket.

Thanks, it was just 5 though and i feel silly doing a ticket if it turns out they were counted after all :) I just didn't know if anyone else had had that happen to them
