I had received this book as a quest prize, and when I clicked on it in my inventory, an error came up saying "item number not found!" (or something along those lines). I thought this was weird, so I went to Lock the item and try it later. When I Locked it, the item disappeared completely!

that's definitely odd… i'd suggest filing a ticket, a UA can look into it and probably give you a new one.
