Has anyone got the achievement for Item Scramble? Unless I misunderstand it, guessing correctly once should award it. I guessed all 25 correctly and never got the achievement.

You have to guess it correctly when you get the challenge mode one. For that one, there's a timer and 6 blanks for you to solve.

If you did do this one, the yeah its a bug because someone in the achievements forum already has this.
