Hey, welcome :) First, nice to see you here, hope you'll enjoy :) You're coming in the middle of a plot! Look this up, it is a very helpful thread about the plot. The smudge on your cheek is the first stage of the zombie disease you've gotten, the Baker's Kiss. Here you can see all the stages. Furthermore, there's the Donation Corner which lets you grab a free item every five minutes and the Free Food station to be found at Riverside Valley, if you need a good point to start. As for your questions...

The smudge is called "The Bakers Kiss" Its a virus that once it reaches stage 6, will let you into the underground and give you access to the zombie den. There's a really nice thread about it here which will explain everything alot better than I can.

As for Skitters and the Journal, if you want to go into a bit more detail, I might be able to help with those too.
