It usually happens when it appears that your post didn't go through or is stuck so you refresh or keep clicking the create post or edit post buttons.

When you make a post with pings in it, and for some reason you're not seeing it process, open a new tab and copy/paste the address from the first tab into the second. Give it a few minutes, then check the second tab to see if it's gone through. The forums are having some issues but pings are usually processing on the first try now :)

[edit] Haha, that literally just happened with this post. I had to open it in a new tab to see if it had processed since it just said "Making your post now..." for a couple of minutes ;)
