When someone presses the ToT link for you, is it possible to get egged or tped instead? I keep posting in the "ToT the person above you" forum, but keep getting egged/tped instead of ToTed.

Well, the buttons are all in a row, if someone's not paying attention, they can accidentally click on the wrong button. However, no, there's no glitch I know of that will egg or TP you instead of ToT.

Could it be that both are happening? You don't get notified that someone trick or treated you - only if they happen to hit the 5th ToT mark. But you always get notified when egged and TPed (which people are doing way more of this year because of new achievements).

Oh! I thought you were notified each time for all three, not just every 5th time for toting. That makes sense then, since it's probably only about every 5th time posting in the forum that I got a notification.
