I'm in the process of moving in with my boyfriend's parents; no one plays Subeta there, but would I need to make a ticket since I'll be on a different IP address Monday afternoon? In about 6 months we'll be moving in our own place then so I need to know so my account doesn't get ban.

If no one else is playing Subeta at the residence, I don't think you need to. I've moved a couple of times since I started playing Subeta and never submitted anything about it and nothing happened. Also plenty of people access Subeta from multiple devices anyway like their phone and laptop or other computer.
I think it's fine if it's only your account, they just want to know if multiple accounts are being accessed from the same devices or similar IPs so they know it's multiple users and not some sort of cheating going on using multiple accounts.

if you want to be sure it's best to file a ticket in help. Most of us are regular users and not staff (although they do pop in).
I have accessed Subeta on multiple devices without logging out and I have also done it from my brother's place four hundred miles away and never had any problem about it.
yeah I wasn't sure on that. I would log in throughout the years to keep my account active so I wouldn't lose my name but that was it.
