How do I combine ingredients together to make things from the alchemy shop? I've got the right token and other items I just don't know what to do next.

go to The alchemist (link), in the Shadowglen underground, you'll need to be wearing a zombie skin to get through the door. He'll show the items you have, and you choose the ones you want to combine.
