There isn't any way to put an apostrophe in Subeta item names (coding issue I think), so "Elmo's Flask" is probably exactly what it is. :)

As for who Elmo is, I don't know for sure but always thought it was a joke about the actual Elmo.
I have never heard of an NPC with that name, I thought it was just a reference to the St. Elmo's Fire phenomenon.
But I could always be wrong.

It's almost certainly a reference to the St. Elmo's Fire phenomenon -- the description is "Contents of this flask are so strong, they may have you spitting fire."
Come on, if anyone on Sesame Street has a secret flask you know it's Oscar the Grouch. :P
