You have to battle with it, and there's only a chance it will make a random pet hungry. I believe it only works on pets other than the one you're battling with (or maybe the effect is more likely on those pets?) I haven't used it personally

I've used that item a bunch and here's how it works for me: A tier 6 pet uses the item in battle and sometimes nothing happens but sometimes in the battle you get a message that Maleria put a curse on you, or something. When you get this message a few things could happen: all pets except the one that is battling with it lose health, all pets except the battler lose happiness, or all pets except the one battling lose hunger. :)
