[edit] Solution is to do a Library quest. :)

Look after the word 'visited.' (click for full view) "Back to searching" leads to the explore map.

Have you tried refreshing (or revisiting her)?

Yep, multiple times and just now, it's still the same. I'm probably just going to click "give up," but I'll leave this here in case somebody could figure out why it happened. :P

I suppose you could just do every quest and see if any of them give you the last piece?

Oh, that's a good idea. I haven't clicked give up yet, so I'll try that now. :)

I see, I thought that wouldn't have been the problem since it still worked after the revamp yesterday. You're right, it worked! Looks like the solution is to just do a Library quest. :D
