I don't exactly have a form for you to use, but you can just put whatever you want to be edited on the pic in your post. I don't charge much at all, just around 20k. If you don't like the edits I've made, please post telling me the things I've messed up on. If I can, I'll try to edit it again and make it what you wanted. :P
