After you use it to get the extra slots, does it disappear? I have never used one before, obviously. xD Help?

No, you would have the slots forever, but to fill them you would need to have an active GA medal.

Yes, they disappear when used. They can also be played with by pets and put in your plushie collection, so careful not to misclick, that would really suck.

The two slots you get can only be used while you have a gold account active, but they don't disappear - you just can't put a pet in them while you're blue and not gold.
