Is it true if you buy a 1 month gold account medal from user shops you can gain a goold account for that month? or is it just an item?

You can gain a gold account for any month that you choose to activate it. It works the same whether you buy from cash shop or from user shops.
When you buy a gold account from the Cash Shop, you actually get a Gold Account medal. It goes in your inventory, and you click it to activate your Gold Account. The medals from the Cash Shop and the ones you find in user shops are the same :)
