They come from random events or you can buy them from other users. Yes, you can only buy one item per invite you have redeemed.

Nope, unfortunately they don't come from quests. If you get one it'll have a note at the top of the page. They're easy to miss though, I find invites in my inventory pretty often that I never saw the events for.
