If someone submitted a reference image with their avatar, will it show when presented to someone for voting? I've not seen anyone's, but I suppose they may not have had one.

yep, if there is a reference pic, it will show next to their avatar when you get to choose. I haven't seen all that many, so it's quite possible you just haven't had any yet.
