Is it possible to change your eyes or your mouth for your avatar?

Yes. Go To The Personal Tab on the top navigation, then click wardrobe and then when it loads, click Avatar on the left :)

There are also wearable items that you can put on your face that will partially or completely cover over your eyes and mouth.
