This is likely due to the caching system running out of memory and deciding to suffer in silence rather than actually tripping any notifications. It should be sorted out at this point, although we are keeping an eye on things to make sure. Please post and ping me if this recurs. Thank you!

Yes, they're part of the same root issue. Basically, the servers that cache information were filling up and didn't have the memory to complete requests. But they weren't actually tripping notifications, so Keith had to specifically check and reset.
