Something went horribly wrong when Rox tried to make and use a Polyjuice Potion. Someone in the other house probably has a griffin, cause she got some of those features. To make things even worse she hadn't noticed her black cat had dropped a hair or two inside the potion too, she looked horrible.

Beth and Lyn planned to change into the Slytherin prefects, hoping they could teach Eros some polite manners as prefects of his house. Oh no! Seems like they’ll have to come up with a new plan.
Looks like the hair strand Beth thought belonged to Sam was actually unicorn hair. It must have fallen on Sam's robes during the last Care for Magical Creatures club meeting.

Magnus tried to sneak into the Hufflepuff house, hoping to play a prank. Sadly someone must have a pet bat. His claws painfully broke through his trainers. He sure wasn't looking forward to telling his parents he needed new shoes.

She's pretty sure she got a bit of bird feather from an owl in the potion, along with whatever hair it was. Should have cleaned her clothes off better after visiting the mews....

The thing about thestrals is that, more often than not, you can't see if a strand of their hair (yes, they have hair--the Elder Wand has a thestral hair core) falls into your Polyjuice Potion.
This poor person has learned this odd lesson the hard way, though she may not ever realize it.
Although she can't see herself in the mirror apart from her floating torn clothes, she can definitely feel that something is drastically wrong with her body. Her skin feels leathery and slick in many areas, and her hair is falling out in clumps.
At least she can hide it.

Trying to sneak into Ravenclaw, Paddy had taken a hair from Fade at the last Care of Magical Creatures club meeting. Unfortunately, it must've been a Unicorn hair from the previous meeting, rather than one of Fade's....

Gaia shouldn't have finished her potion at the owlery. It seems the owls flying overhead left her a feathery present. So much for sneaking to Hufflepuff and taking their food.
