Mine goes that I'd prefer an anthro to a human. As impossible as that is. But Yew-4(i.a.) is just one, I've developed many.
To me its nails on a chalkboard. I grew up around people who revered these types.
I actually don't mind house centipedes, even silverfish don't freak me out. For some reason, it's only earwigs that evoke that response although I've never seen a regular centipede (IRL) before and one of those might set it off. Otherwise, it's just so bizarre to me that I have this issue. I've always been one of those people who doesn't have a problem either catching bugs to let them outside or smushing bugs to put them in the trash, and if it comes to non-bugs? Like rodents or snakes or whatnot, I'll grab them barehanded (assuming the snake isn't venomous, obviously). It's just... something about their nasty little bodies and weird pincer tail... shudder

Dishes in the sink that haven't been cleaned or washed off. I hate touching and hand-cleaning dirty dishes, especially when someone else used the plates and silverware (but of course, I still do it anyway). I just shudder while I'm cleaning them.
Insects and spiders (with emphasis on spiders). Usually the sight of them gives me an uneasy shudder. I just did thinking about them lol.
And I 100% agree with everyone who said practically anything at the dentist. The worse is getting a filling done, but even simply getting them cleaned. Gosh, I can't stand it.
-Scraping most things with my nails. -A stranger being close to me and, a lot of times, looking at me. If someone touches me, that makes the shuddering 1000x worse. -Loud, high-pitched, and low-pitched noises. -Onion chunks. -Certain fabrics and fabric combinations. I know leather and sometimes, cotton and polyester, make me shudder.
Onion chunks eh? When I was a kid, i went to subway and ordered a plain turkey sub. Somehow a chunk of onion snuck into my sub and I think i vomited in the sink. I still dont like uncooked onions, i like them sauteed.

- Oh gosh, that's awful. D: As someone who worked at Subway, it can be very easy to get ingredients mixed in together, especially since they value workers making subs fast. I tried to minimize this from happening, but I was slower as a result.
I've had stray onions in my food before, and I've lost my appetite. A lot of times, if I say "no onions", there's still onions. Now, I just say I'm allergic to them. XD The only ways I can have onion are as a powder and if they've been essentially dissolved in soup and not noticeable.
My biggest shudder is silverware scraping onto ceramic plates. In general, silverware seems to cause it for me. Even though I use it fine, seems like the slightest wrong bite-down gives that "metal in the mouth" feel to me.
people spitting or saliva in general is soooo gross to me. if i see a baby drool i literally have to leave to stop myself from throwing up!!!!! i dont know why im so disgusted by it :/
i also hate hangnails and ingrown fingernails. they bother me forever and i just kind of freak out when i have em.

I dont think ive ever successfully bitten down on my silverware, but i DO hate it when a fork is scraped across ceramic plates, eyyug.
I also do not like spit or drool but im gonna have to get used to that one as i will be becoming an RN, and eventually a hospice nurse. Ill have to deal with every fluid imaginable. Im definitely not looking forward to that projectile vomit and diarrhea, im hoping to leave that to lower tier nurses and interns!
I totally get everything on your list, however when it comes to wet food, im a-ok with it being wet bread as long as my wet bread is the product of a broth soaking into it, especially Italian beef au jus!

-Insects that bite, sting or stink. Bonus points for the ones with too many legs. (Centipedes!!!) -Cigarette smoke. The smell makes me feel sick even if I'm just behind a smoker for a few minutes when I'm stuck in traffic. -Finger tapping. Most nervous habits don't bother me but this one...I want to slap the person's hand even if they're a complete stranger. -Being put on hold and then listening to really bad elevator music. I loathe calling tech support or customer service of any kind. -People who want to argue or tell long stories over the phone. -Raw onions no matter how finely they're chopped. I hate the taste and texture of them. -The loud beeping trucks make when they back up. I used to live above a road that got plowed in the early morning hours. Sometimes they would scrape ice too. Road construction signs are also incredibly annoying. -Most other drivers. Some of the things I've seen on the road just defy all logic.

The only time i dont like beeping back up noises is when im in a store and people on those scooters, usually just walmart, are super loud and obnoxious with them. Hurts my ears and makes me uncomfortable.
Funny you mention you dont trust the dentist, my dad told me a story once about getting his wisdom teeth removed as a kid. He wouldnt let the dentist knock him out because he thought he was going to be diddled, so they numbed his gums up real good and shattered his molars just like that, while he was wide awake. My dad said the noise was something else, that it rattled your whole brain, hearing them crack and shatter and it shockwaved through his whole head and eardrums. Id probably do the same thing since i dont like being drugged, but thankfully my dentist said my wisdom teeth dont look like theyll need to be removed.

Eggs. Even writing that word was gross. I hate the thought of them, the sight, ick.
i thought me and my partner were the only ones who cannot eat onions. under any form or shape. the smell of yellow onions makes us both puke, almost. ewww i can eat onion only if it was super over cooked that i can't taste it anymore - mother can do that, but even so, i try my best to stay away from it. shudders..
i get sad when i discover people who don't like mayo. :( but since it's not the healthiest food around, i guess, i can understand... [maybe you didn't have good mayo to begin with....? eeh, don't worry, i'm not forcing my mayo love on anyone, lol]
other things that make me shudder: -garlic - just no, burn it! -roaches [i'm slowly desensitizing, but still] -rats. mice look cute, but they're still gross -ticks [oh gosh, i can barely see their name written. my vet has a HUGE poster with one on the wall: i nearly puked]
i'm lucky as i'm not that easily grossed out, and i rarely come in direct contact with any of these things [except roaches]

I hate the feeling of being wet without being in water - i.e., you get caught in a rain storm, or step in a puddle, etc. Taking a shower/swimming is OK. walking barefoot through wet grass? Not so much.
Eggs because they're gross and smell like farts. Celery is gross and serves no purpose.
I used to fly a lot for work and people would use the aircraft lavatories barefooted or in socks. That's just rank on another level.
