Yep this is the official thread for HA identification (as it hasn't hit 1k posts yet & when it does a new one will be created). It is possible that no one saw this posted (HA is not an extremely active forum/thread) or no one knew what the item was as it may be a bit obscure! I'm sorry you are still unable to find the item.
Right now I can suggest looking at items under the body, necklace, or possibly even shoulder layers for the tattoo!
what is the context of where you found this avatar? Looking at the url it looks like it might have come from the old EPICon Cosplay Contest, if it was the 2010 one, that was back in the day before CWs existed and users could upload their own tattoos. There's a chance that this isn't a real item.
